POLICIES
If a patient needs to Reschedule or Cancel an appointment please do so at least 2 hours in advance.
The online scheduling app will allow you to cancel or reschedule your appointment as needed. A patient who reschedules or cancels less than 2 hours in advance is subject to a charge of 50% of the cost of the visit charge as stated on the website.
Excessive rescheduling or canceling of an appointment (more than 2x per month) may result in the discharge of the patient from care with a referral to a different provider.
NO SHOWS
A patient who fails to cancel or reschedule an appointment online or without contacting me via text or phone and leaving a message may be charged for the full price of scheduled service.
